I understand that completing a Coaching Application with the East Gwillimbury Minor Hockey Association (EGMHA) does not quarantee me a coaching position with the EGMHA.

I authorize EGMHA to collect personal information appropriate to the position applied for by verifying the personal character references I have supplied.
I also understand that in order for my application to be considered, I must complete a Police Vulnerable Sector Check, unless a valid PVSC is already on file with EGMHA. If expired a new PVSC must be submitted.
