Thank you for completing the Exhibition Game Permit, it has been saved in our database and sent to the EGMHA Admin/Registrar for review.  Once approved by the OMHA, the permit will be sent to the email address entered in the form.

Note: All exhibition games being played outside of the OHF will incur a $10.00 fee.  After the fee
          has been submitted to the EGMHA Admin your permit will be forwarded to you.
