I authorize East Gwillimbury Minor Hockey Association to collect personal information appropriate to the position applied for by verifying the character references I have supplied.
I also understand that in order for my application to be considered, I must complete a Vulnerable Sector Screening report with York Regional Police (if there is not one currently on file with EGMHA).
I further understand that all coaching positions require that minimum certification requirements be met and that I am responsible for obtaining this certification by August 31, 2023. It is also understood that by signing this application, I agree to abide by the Code of Conduct, Constitution, Bylaws, Policies and Playing Rules of the EGMHA/OMHA/OHF and that I am subject to discipline or suspension at the discretion of the EGMHA Board of Directors.

The personal information collected on this form and on any documents collected by EGMHA will be used for the sole purpose of administering the Rules, Regulations and the By-Laws of the EGMHA/OMHA/OHF and/or for the purpose of administrating the coaching selection process.
